You may need to order a replacement MedicAlert® Emblem due to a change or addition to your medical conditions, or you may just want to upgrade to one of our range of precious metals. Whatever the reason, you can place the order using one of the convenient methods below.

Please note, if you have previously had a large disc, it is advisable to order a similar size for your new Emblem. The size of the disc effects the the amount of information that can be engraved. Bracelet discs: a small disc can carry approximately 4-6 words, standard disc 5-7 words and large disc 7-11 words. Necklet discs: small disc 7-9, large disc 8-10 words.

Once you have made your selection from the Product Range section of our web site, you can order your new MedicAlert® Emblem quickly and easily by calling freephone 0800 581 420 during office hours, Mon-Fri, 9am-5pm. The Membership Services team will take your payment by credit or debit card and record any changes or additions to your personal details. One of our fully qualified nurses will then take down any new medical information.

If you would prefer to order by post, a colour replacement order form can be obtained by calling freephone 0800 581420. Alternatively, you can download and print a PDF version of the form by clicking here. Once you have completed the form, please send it, along with the relevant payment, to MedicAlert® at, 1 Bridge Wharf, 156 Caledonian Road, London, N1 9UU. If you have any changes to your file, please send the details on a seperate sheet.

Re-engraving an Existing Precious Metal Emblem

Please note that if you already have a silver, gold-plated or gold Emblem that requires updating due to a change or addition to your medical details, this needs to be returned to us for re-engraving. The prices for this are detailed below:

Silver / Gold-Plated - £35
Gold - £50

In order to guarantee safe delivery, we recommend returning precious metal Emblems by Registered Post.

Repairs

If your current MedicAlert® Emblem is in need of repair or alteration, please send it in to the above address with a covering letter and we will return it to you as soon as possible. Although there is no charge for this service, as a Registered Charity, a donation would help contain costs.
