MedicAlert is unique in being able to register an individual's Advance Decision and communicate important personal wishes regarding treatment in an emergency.
If you have an Advance Decision and wish to register this information with MedicAlert, please click here to join as a new Member.
As part of the application process, we will ask you to provide further information about your Advance Decision. Should we require any further information from you, we will contact you as soon as possible.
If you are an existing Member, please call Tel. 0800 581 420 to find out how you can submit your Adanced Decision to be included in the MedicAlert system.
