To ensure that your helmet tag is as effective as possible and works well with your medical record we recommend the following approach to completing the paper slip that goes in the helmet tag:
1. Add your MedicAlert Member ID (starting with GB).
2. Write only the most important information on the slip. This is likely to be the information engraved on the back of your MedicAlert ID (your bracelet or necklace).You should avoid including medication on the form, unless it is likely that this medication may be relevant in an emergency.
3. Once your form has been completed, simply fold it up and place it inside of the helmet tag and tuck in the flap. Finally, using the 3M adhesive strip, stick the helmet tag to the side of your helmet.
Please remember that this helmet tag is not intended to replace your MedicAlert ID, as it is only useful when you are wearing your helmet. It does, however, provide the most important information, when your MedicAlert ID may not be visible or accessible due to heavy clothing.
We guarantee that your medical ID jewellery will be made to the highest standards by our expert jewellers and it will be delivered to you in pristine condition.
MedicAlert provides a Warranty on all manufacturer defects from the dispatch date as follows:
The Warranty covers: defects in the material or workmanship of clasps, chains, straps or wires, watch batteries and the readability of the disc.
The Warranty does not cover: repairs made by anyone other than our skilled jewellery team; products damaged from everyday wear and tear; tarnishing of silver; scratching of the disc, enamel, chain and attachments; damage resulting from misuse or accidental handling; exposure to lotions and creams; lost/misplaced products; and changes in engraved information after the engraved product is approved.
In the unlikely event that you have a problem with the jewellery within the warranty period, you can return the item to us for inspection and we will either repair or replace it.
Your statutory rights are not affected by this guarantee.
However, sometimes we may need to contact you to check some medical information which can cause a small delay if we cannot reach you.
