First year fully funded membership!

For those living with dementia, we are able to offer the first year of MedicAlert membership for free. This is kindly sponsored by The McLay Dementia trust, with MedicAlert match funding the programme. 


This sponsorship supports those living with all types of dementia, including Alzheimer's in accessing the MedicAlert Safe and Found programme. Launched in partnership with Thames Valley Police.

What do you need to do?


This programme is open to anyone living in the UK or Republic of Ireland who has been diagnosed with dementia or another cognitive or memory loss condition.

Purchase your choice of MedicAlert ID. Worn at all times, this enables access to your MedicAlert record.

Store a Herbert Protocol

To help the police if you are reported missing, you'll need to submit a Herbert Protocol within 2 weeks.

What happens now?

Membership

If you have submitted your application form, you have been added to the waiting list. You will be notified once your application has been processed and approved. We aim to be as timely as possible.

Herbert Protocol

You can now submit your Herbert Protocol if you haven't already. If have not recieved your membership number, you can still complete and submit the form, we will add this for you later.

Nurse checks

Once you have submitted the form, if anything is missing, you have not completed the application form or signed up as a member, one of our nurses will make contact with you to complete this.

MedicAlert ID

Once you have recieved confirmation of your successful application, we will provide you with your membership details and assist you in purchasing your MedicAlert ID.
