The MedicAlert Foundation UK and the Coalition for Personalised Care (C4PC) have partnered to support people living with health conditions to have greater choice, confidence and control in managing their care. Together, we aim to support people to take a more active role in their health and wellbeing, while ensuring important medical information is available when it is needed most.
C4PC is a movement of health providers, commissioners, people with lived healthcare experience, and their representative bodies. Their vision is to make personalised care an everyday reality for people who rely on healthcare services, as well as organisations & people providing them.
Through the partnership between MedicAlert and the C4PC, MedicAlert members can benefit from a more personalised approach to managing their health and care. Personalised care encourages people to be involved in decisions about their health, helping them feel more informed, confident and in control.
For MedicAlert members, this can support better access to care, improved health outcomes and greater understanding of how to manage their health. By having important medical information recorded and accessible through MedicAlert, members can also feel reassured that healthcare professionals have the information they need to provide more informed, person-centred care when it matters most.
C4PC creates impact through three key areas: sharing knowledge, supporting people and organisations, and shaping change.

The 2025 - 2026 strategy for the Coalition for Personalised Care (C4PC) was developed collaboratively with a wide range of partners, with a focus on making personalised care more effective and accessible. Its key objectives include improving communication, promoting greater understanding of personalised care, and increasing health equity so that everyone has the opportunity to receive care that meets their individual needs.
