Membership provides you or you child with a personal online record to securely store medical information, including medical conditions, allergies, medications, history, emergency contacts and scanned documents, alongside directives or precautions to ensure no time is lost in an emergency.
You will also receive a piece of medical ID jewellery for free. The disc on the jewellery will be engraved with the most vital information, your or your child's unique membership number and our 24/7 emergency helpline, so that first responders can access the full online record if necessary.
If you have been handed information from either Supporting children and young people at with The Children's Heart Federation (CHF) The Children's Heart Federation, you are eligible to apply to this scheme for yourself or your child.
Places for the scheme are limited and may not always be available due to funding. You may be placed on a waiting list if no spaces are available at the time of your application.
