Consent to Use of Image and Recording

I give my explicit and informed consent for MedicAlert UK to record, use, reproduce, and share photographs, video footage, and audio recordings of me for promotion, public relations, education and marketing.

This includes (but is not limited to) use in the following channels:
- Broadcast (TV, YouTube, GP TV, Freesat, BBC iPlayer, etc.)
- Social media platforms
- Websites, including www.medicalert.org.uk
- Printed and electronic publications
- Outdoor media, including:
  - Billboards
  - Posters
  - Public transport ads
  - Public event displays and signage
- Partner organisations' promotional use

Data Protection and Rights (UK GDPR)

In accordance with the UK General Data Protection Regulation (UK GDPR) and the Data Protection Act 2018, I understand:

- MedicAlert UK will lawfully, fairly, and transparently process my personal data (images, video/audio recordings).
- The data will be used solely for the stated purposes and stored securely.
- I have the right to:
  - Withdraw my consent at any time by contacting info@medicalert.org.uk
  - Request access to the data held about me
  - Request correction or erasure of data, where applicable
- I understand that withdrawal of consent will apply to future use only and may not affect materials already published or disseminated.

Permissions and Waivers

- I waive the right to inspect or approve the finished materials where my image or voice appears.
- I waive any rights to royalties or compensation for use of these materials.
- I understand that the content may be edited, copied, exhibited, published, or distributed without further notice.
- This release is valid indefinitely and without geographic restriction, unless I withdraw consent in writing

For Individuals Under 18

If the subject is under 18 years of age, this form must be approved by a parent or legal guardian.
