The Wallet Card is a plastic, credit-card-sized medical ID card that displays essential information from your MedicAlert medical record. It’s designed to be carried easily in your purse, wallet, or MedicAlert Document Holder.
The front includes: • Your name • Your MedicAlert GB membership number • Your primary contact number • An emergency contact’s name and number • Your GP’s name, clinic/surgery name and phone number • Your hospital name and phone number
The reverse side lists details from your MedicAlert medical record in this order: • Conditions & implants • History of Anaphylaxis • Allergies • Medication • Miscellaneous notes If your full record cannot fit on the card, a note will indicate that additional information can be accessed through the emergency helpline number printed on the card.
